Here's what you do next:

• Complete the application for residency
• Mail the application for residency to Parkview Village in an envelope. Please be sure to include payment for the deposit at this time too.
• Once your information and deposit have been received, you will be added to a waiting list and someone from Parkview Village will be in touch with you to coordinate your move in.
• During the assessment, you will be provided a physical form to take to your doctor.
• Decide when you would like to move in!
